POSITION SUMMARY: This is a Registered Nurse position responsible for providing public health nursing care and case management to individuals, families, and the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Provides public health nursing care and case management to individuals, families and the community throughout the health/illness continuum in the home and community.

• Provides health education about disease and prevention of disease to individuals, families and the community.

• Teaches recognition of early symptoms of communicable disease and the importance of immunizations.

• Assists with conducting immunization clinics as appropriate.

• Works with tribal, state and county health agencies to address health needs of community

• Collaborates and coordinates services with medical and other professional personnel in the SUHC working as an integral team member to manage care of clients at Southern Ute Health Center.

• Collaboration and coordination with other resources such as tribal CHR program, and other home care agencies that service the community.

• Maintains records and reporting systems according to policies.

• Appraisal of health care needs and assistance with surveys as delegated.

• Maintains a caseload of clients and is able to prioritize and schedule them independently.

• Participates actively in quality improvement projects for the community health nursing program.

• Is self-motivated in planning and scheduling and requires minimal supervision while on visits and in the community.

• Shows sensitivity to cross-cultural and language differences of the various patients served.

• May be required to be on call or required to work additional workday hours as necessary.

• Complies with Tribal and other appropriate Policies, Procedures, Rules, and Regulations.

• Performs other work-related duties when required or requested.

QUALIFICATIONS:

Must have an AA/AS or Diploma in Nursing from an accredited college or university.

Must maintain unrestricted licensure as a Registered Nurse in the State of Colorado.

Must have 1 year of specialized experience which includes delivering health and nursing care to individuals, families and schools within communities and using initiative and judgment in covering a range of nursing services through counseling, teaching and follow up within established plans, policies and objectives of the agency.

Must maintain basic life support certification.

Must have computer experience in software programs including Microsoft Word and Excel.

Must provide current immunization records or obtain required immunizations at the Tribal Health Center prior to starting work to meet regulations required for all clinical providers.

Must provide recent TB test results or be able to obtain a TB test at the Tribal Health Center prior to starting work.

Must have a valid Driver’s License for state of residency and be insurable under the Tribal Vehicle Insurance Policy.

Must pass a thorough criminal history background check, fingerprint check, and pre-employment drug test.

PREFERRED QUALIFICATIONS:

General knowledge of Indian Self Determination P.L. 93-638

Current Advanced Cardiac Life Support (ACLS) certification.

Prior work experience with Native American Indian Tribes.

TERO Preference.
