Position: Pharmacist

What You'll Do:

As a pharmacist, you will provide comprehensive pharmaceutical care through integrated drug distribution, clinical intervention, staff education, training, clinical services, and continuing education. This role involves supervising pharmacy technicians and intern pharmacists.

What We're Looking For:
  • Experience:
    • ASHP accredited residency or significant clinical experience is preferred.
    • Meditech experience preferred
  • Skills:
    • Reviewing patient medication history
    • Educating patients on drug therapy
    • Establishing desired outcomes and therapeutic plans
    • Assessing patient demographics and developing drug monitoring plans
    • Implementing and monitoring new drug regimens
    • Interpreting laboratory data and reviewing patient medication profiles
    • Monitoring pharmacokinetics and potential adverse drug-drug and drug-food interactions
  • Education:
    • Pharmacy Doctorate degree
  • Licensures/Certifications:
    • Current California Pharmacist License
    • LA City Fire Card certification (to be obtained within the first 30 days of employment)

Specific Responsibilities:
  • Clinical and Therapeutic Care:
    • Maintain competency in unique aspects of drug therapy for all patient groups.
    • Monitor drug therapy for appropriateness, dose, regimen, route, therapeutic duplication, and drug interactions.
    • Conduct target drug programs and drug usage evaluations; report results to the Pharmacy and Therapeutics Committee.
    • Detect, document, and report adverse drug reactions and medication errors.
    • Promote formulary use and manage procurement of non-formulary drugs.
    • Maintain and update patient profiles with demographics, diagnoses, allergies, and current medications.
    • Discuss medication order clarifications with prescribers and document changes.
  • Educational and Training Responsibilities:
    • Provide presentations and publications on drug-related topics to the healthcare community and general public.
    • Participate in continuing education programs, meetings, and training.
  • Operational Duties:
    • Ensure proper technique and accurate preparation of pharmaceutical products.
    • Oversee the timely and accurate dispensing of drugs and solutions.
    • Provide drug information to healthcare professionals and patients.
    • Perform computer order entry for IVs and other medications as needed.
    • Ensure compliance with controlled substance distribution and control systems.
    • Participate in unit inspections and other pharmacy areas as required.
  • Documentation and Compliance:
    • Document clinical interventions and cost avoidance efforts.
    • Ensure compliance with National Patient Safety Goals and hospital policies.
    • Participate in performance improvement activities and infection control measures.
    • Maintain confidentiality of patient records and information.
Work Environment:
  • Setting: Primarily indoor building/office environment, well-lighted and ventilated, with possible multiple work sites.
  • Pace: Fast and continuous work pace with a variable workload.
  • Interactions: Frequent contact with staff and public under varied circumstances. Requires clear communication in English, both verbally and in writing.
  • Interruptions: Subject to interruptions from calls, inquiries, and potential emotional situations.
  • Emergency Situations: Handles emergencies/crisis situations according to hospital policy.
  • Travel: Occasional travel may be required.
  • Hazard Exposure: Potential exposure to chemicals, flammable materials, gases, electrical/radiant energy, or equipment with/without moving parts.
Physical Demands:
  • Continuous (66 to 100% of time): Standing/walking, fine motor activities, near vision, hearing, and verbal communication.
  • Frequent (33 to 65% of time): Reaching above shoulder level, forward bending, twisting, squatting, and kneeling.
  • Occasional (0 to 32% of time): Sitting, climbing, lifting/carrying up to 25 pounds, repositioning/transferring patients up to 200 pounds, pushing gurneys, wheelchairs, and other medical equipment.
