Job Description

We’re seeking a dedicated and compassionate Community Outreach Worker to join our team and help connect vulnerable individuals with essential health and social services.

What You’ll Do

As a key member of our Outreach Services Team , you will:

  • Engage directly with adults experiencing homelessness in shelters, on the streets, and other community settings.
  • Connect clients to critical services like healthcare, mental health care, and substance use treatment.
  • Provide one-on-one health education and risk reduction interventions, including the safe use of medical supplies and naloxone.
  • Support clients by accompanying them to appointments and helping them navigate services.
  • Collaborate with a multidisciplinary team to develop holistic care plans tailored to each individual’s needs.
  • Assist in mobile clinic operations and street outreach events.

What You Bring:

  • A high school diploma or GED (Associate’s degree in Human Services preferred)
  • At least 2 years of experience serving hard-to-reach populations
  • Strong knowledge of Baltimore City resources and a commitment to harm reduction and trauma-informed care
  • A personal vehicle, valid Maryland driver’s license, and a willingness to travel throughout the city
  • The ability to work comfortably in unpredictable, street-based environments
