This Home health position allows you to be out in the community, enjoying the weather rather than being in an office or hospital and provides flexibility to schedule patients on a daily basis.
~ Schedule: M-F 8-4:30 with 2 times a month weekday call and 1 weekend day every 6-7 weeks. Either a Saturday or a Sunday.

At SSM Health, you can expect a work environment where:
• Provides care to patients under the direction of a registered nurse or physician, functioning within the scope of license.
Delivers care to assigned clients and performs direct care in accordance with an established plan of care.
Delivers safe care at all times that is in line with physician orders, home health policy and established nursing care standards.
Maintains communication regarding patient progress with physician, supervisor, staff and family members as appropriate to the situation. Reports patient-related observations to supervising RN on a daily basis.
Provides documentation that follows the established treatment plan, supports coordination of patient care, meets regulatory requirements and ensures reimbursement.
May perform point of care testing according to policies and procedures.
Graduate of an accredited school of nursing or educational equivalency for licensing
One year licensed practical nurse experience
Frequent sitting, reaching and keyboard use/data entry.
Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA)
Licensed Practical Nurse (LPN) - Illinois Department of Financial and Professional Regulation (IDFPR)
Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA)
Licensed Practical Nurse (LPN) Nurse Licensure Issued by Compact State
Licensed Practical Nurse (LPN) - Missouri Division of Professional Registration
